Appointments

We request that you arrive on time to your appointment. Please bring your insurance card and copay (if you have one) to every visit. If you are a new patient we will send you information in the mail-please fill this out before coming to your first appointment. Also, you will need your insurance card, copay and your drivers license or picture ID.
If, for any reason, you cannot keep your scheduled appointment we simply ask you to call us. We are usually very accommodating to reschedule that appointment. Our policy is to charge a fee for No Show appointments if you do not call us at least 24 hours prior to your appointment time. We will charge $30 for appointments and $100 for Office Procedures.

After Hours & Emergency Care

If you have an issue that cannot wait until normal office hours please call our office number (616) 247-1700. The answering service will get a hold of Dr Dalm and take care of you. For an emergency that requires hospital care please go directly to St Mary's Hospital and they will take care of you. However, if your issue can wait for our office staff please call during normal business hours.  

Prescription Renewals

Please call our office at (616) 247-1700.  We ask for 24 hours to fill your prescription.  You will need to supply your name, date of birth, a number to reach you at, prescription name, how you take your medicine, and which pharmacy you would like it called into. If you have the pharmacy number please leave that too.  

Financial

Please come to your appointment prepared to pay your portion. If you owe a copay for your visit, it is required before you see the doctor. If you have insurance we will bill it for you as a courtesy but you must supply us with the proper information.
